Section I & Section II must be filled out completely by Minor/Employer in order for Section III to be completed by school official.
All instructions are on the back page of permits.

When CHS Office is closed:
- Print off form and work on getting Section I & Section II complete.
- Please email Mrs. Kolf or call and leave a message concerning work permit completion.
